THE REFERRAL PROCESS

1.        Physician’s office to fill out referral form with physician signature. 

·        Attention:  Intake Department/Pharmacy  

·        Phone:  1-866-699-4573

·        Fax:  1-618-443-2547  (Please call prior to faxing.)

 

2.        Health Options will:

·        Verify insurance benefits

·        Obtain Prior Authorization

·        Speak with Case Management if required.

 

3.        Health Options will:

·        Call physician office with patient status update (ie. Patient approved for therapy or certificate of medical necessity required, delay in authorization, medical review, etc…)

·        Verify patient’s schedule for injection appointments for entire season.

·        Fax authorization and verification of patient acceptance to physician’s office.

 

4.        Health Options will bill insurance for medication.

 

5.        Physician will direct treatment and injection setting.
